Headteachers are only able to grant a leave of absence during term time in exceptional circumstances. If you feel this applies, please read West Berkshire’s Holidays in Term Time – A Guide for Parents before submitting a request to the relevant school(s) using the form. You are advised not to make any booking before ascertaining if the proposed holiday has been authorised.
If your child needs medication administered during school hours, please complete this form and return to the school office. All medication must be in its original packaging with the child’s name on it.
